  • Applicant must be age 65+ or disabled to qualify
  • You must live inside the city limits of Red Bank
  • If all spaces are not completed, the form will be returned
  • Only one person per application
  • Do not Attach Transportation Requests or Schedules
  • Application may require 10 days of processing time. If a decision is not made within 10 days, please call our office at 423-698-9038
